Physician Membership Dues: – $125.00 per year
Fellow Membership Dues: – $125.00 per year

Ph.D., Nurse, Physician Assistant, Social Worker, Physical Therapist, Nutritionist, Dietician
Non-Physician Membership Dues: – $75.00 per year
